CASE 06

Medical Writing & Protocol Strategy for a European Cardiovascular Program

Integrated scientific, statistical and regulatory review before protocol finalization.

Case Overview

Why this case mattered

A European pharmaceutical sponsor asked TUNECT to review an interconnected cardiovascular development program before protocol finalization. The task combined Medical Writing, Biostatistics and regulatory reasoning rather than treating the protocol as a purely editorial document.

Multi-study review

linked designs assessed as one development program

Evidence-based

reference trials and regulatory guidance reviewed

Protocol-ready

recommendations structured for synopsis and protocol refinement

The Challenge

What TUNECT walked into

The proposed later-phase studies implied large sample sizes because clinically meaningful thromboembolic events were expected to be relatively infrequent. The Sponsor wanted to understand whether the program could be made more feasible without weakening the scientific question or creating a less defensible regulatory position.

Our Approach

How the project was handled

01

Program-level review

Reviewed linked study designs and sample-size assumptions together rather than assessing each protocol in isolation.

02

Evidence + guidance

Compared assumptions with pivotal reference trials and relevant European regulatory guidance, including endpoint and non-inferiority considerations.

03

Protocol strategy

Evaluated event-driven planning, blinded sample-size reassessment, longer follow-up and risk-enrichment options, and translated conclusions into protocol-ready recommendations.

Medical Writing is strongest when protocol language, statistical assumptions and regulatory rationale are developed together rather than handed off sequentially.

Outcome

What changed

Better design logic

The optimization discussion shifted away from simply weakening endpoints or margins.

Explicit assumptions

Key event-rate and sample-size assumptions were challenged and documented rather than carried forward untested.

Writing-ready output

Recommendations were structured so they could be incorporated into synopsis and protocol refinement.
